In a 1978 article I described a chain of three outcomes. You might want to consider them in your future writings and assessments.
1. Immediate outcomes= the actions/activities of of the foundation; sometimes called process outcomes
2. Intermediate outcomes = the immediate results of a foundation’s actions/activities such as the # of grants awarded
3. Ultimate outcomes = the impact intended such as grantees actually making a positive and noticeable difference somewhere in the world on some problem.
I regret to say that so far my assessment of foundations (and of their grantees) is that they are at levels 1 and 2.
